PLEASE READ CAREFULLY. You must complete the following steps to receive your travel award, as well as verify your invitation. Once you have completed these steps, you will then be able to arrange your travel for the Pro Tour Tournament that you won the Qualifier for.

a. TAX FORM - W9 for US & W8BEN for International winners. If you did not receive this form at the qualifying event, you can download it from www.irs.gov/formspubs/. Please be sure to include your full legal name, address, social security number (tax payer ID#), and your signature.
b. CONSENT AND RELEASE AGREEMENT FORM. - We must receive a signed Travel Consent and Release Agreement. If you did not receive the form at the qualifying event, please go here: http://www.wizards.com/dci/downloads/ConsentForm2009.pdf
c. IDENTIFICATION – We must receive a copy of an age verification document. Example: Drivers License, Passport, Birth Certificate, or Gov’t ID.
NO TRAVEL AWARD WILL BE ISSUED UNLESS WE RECEIVE THE ABOVE MENTIONED DOCUMENTS.

CONTACT THE TRAVEL DEPARTMENT
The DEADLINE for setting up travel is 30 DAYS prior to the date you travel to the Pro Tour. Any travel that has not been issued prior to this date will be forfeited.
Once all documents have been sent into Wizards of the Coast, and all event standings have been verified, you will be sent a confirmation email with further instructions on contacting our Travel Dept. You must contact the Travel Dept. via email (preferred) at travelservices@hasbro.com, or call (888)-404-8490 or (401)-431-8490 8AM – 4:45PM US EST, M-F.
NOTE #1: Minors who win a Qualifier are not eligible to be awarded the plane ticket travel award. Instead, minors will be awarded a “Cash Equivalent Award” equal to the cost of a round trip ticket to the Pro Tour in question, as determined by Wizards in its sole discretion. This award will be issued, only if the player actually shows up to the Pro Tour in question on his or her own, and register to play. We will pay out this “Minor Qualifier Travel – Cash Equivalent Award” after the Pro Tour along with the rest of the Pro Tour payouts.
NOTE #2: The travel award prizes handled by the Wizards of the Coast Corporate office, apply only to North & Latin America, & Japan. All other countries handle their travel awards differently. Important - The receipt of this letter does not itself constitute an invitation. An official invitation list for this event can be found at www.wizards.com/protour. Invitations will not be awarded until complete event results are received and reviewed. If a tournament official incorrectly or improperly distributes this letter for any reason, Wizards of the Coast and the DCI reserves the right to withhold any and all invitations and prizes for this event. Invitations may be rescinded by the DCI upon investigation.
